Abstract
Amyloid-β (Aβ) confirmation is increasingly important in early Alzheimer’s disease pathways, but amyloid-PET imaging remains costly and difficult to scale. Recent PET-guided knowledge distillation has shown that PET-derived supervision can be transferred to an MRIonly student model for Aβ status prediction, preserving PET-free inference and avoiding non-imaging covariates such as APOE genotype or cognitive scores, which are often unavailable in routine clinical workflows. To support clinical translation beyond binary classification, this paper presents a clinician-informed translational roadmap outlining three extensions to an existing PET-guided MRI-only framework: continuous Centiloid-aware amyloid burden estimation, longitudinal conversion and prognosis modeling, and region-level interpretability. For each direction, the proposed output target, clinical use case, required data structure, validation metrics, and major confounders are defined. The proposed extensions define a structured design and evaluation plan for future MRI-only amyloid assessment systems, clarifying how PET-guided MRI models can move from binary screening toward outputs that are interpretable, clinically meaningful, and suitable for prospective validation.
